Services may be covered in full or in part by your health insurance plan. Please contact your health insurance company to verify your coverage and benefits prior to your first appointment. Your insurance card will typically provide a phone number to call on the back of your card for benefits inquirires. Please check your coverage carefully by asking the following questions:
-Do I have mental health insurance benefits?
-Is Janette Concepcion, Ph.D., LP considered in-network for my insurance carrier for outpatient mental health benefits?
-What is my deductible and has it been met for this calendar year?
-Do I have a co-pay for each session, or do I have co-insurance (a percentage of billed charges) for each session?
-How many sessions per year does my health insurance cover?
-What is the coverage amount per therapy session?
-Is approval required from my primay care physician?
Any costs of treatment not covered by insurance are your financial responsibility.
I accept the following insurance plans:
Blue Cross Blue Shield
HealthPartners for 3M contract only.
Union worker healthcare plans may be covered if under a Blue Cross Blue Shield contract
Reduced fee services are available on a limited basis for those without insurance benefits.
Cash, check and all major credit cards are accepted for payment. Some insurance plans also have a "Benny" card which is attached to a healthcare savings account (HSA) which can also be used for payment.
Any appointment set up for you is specifically set aside for you. If you do not show up for your scheduled therapy appointment, or you have not notified me at least 24 hours in advance, you will be required to pay a late cancellation/failed appointment fee of $75. Insurance will not cover this charge.
